Please use this form to provide us with information about any such instance(s) that took place outside of the All of Us Research Program. All of Us retains the discretion to make decisions about access that take into account the following information. This may include, but is not limited to, the following actions: reaching out to the entity that issued the penalization to gather more information or contacting other data users if they are implicated in the reported misconduct but have not reported it to the All of Us Research Program themselves.
